Deep Brain stimulation
DBS was introduced as treatment of PD byBenabid and colleagues. The advantage of this method over surgery is that brain tissue is not distroyed. The probable mechanism of action of DBS is release of inhibitory tranmitters, depolarisation blockage jamming of electric signals and backfiring. The most dramatic effect of stimulation were an increase in ontime. The procedure is most useful in treating motor complications in PD.Globus palidus internus and subthalamic nuclei are the most important targets of DBS.It is important to understand that the stimulation did not provide antiparkinsonian benefit superior to L.Dopa, but the benefit is primarily related to redustion of dyskinasia and offtime.Complications of the procedure are intracerebral hemorrage, hemiparesis, convulsions and infections which all are rare.
